Evalue billing provides medical billing services to healthcare providers, including but not limited to claims submission, patient billing, and revenue cycle management. The scope of services will be defined in a separate agreement between Evalue Billing and the healthcare provider.

Accuracy of Information: You agree to provide accurate and complete information required for us to provide our services. Inaccurate or incomplete information may result in delays or errors in billing.
Compliance with Laws: You agree to comply with all applicable laws and regulations in connection with your use of our services.
Confidentiality
We are committed to maintaining the confidentiality of your information. We will not disclose your information to third parties except as required by law or as necessary to provide our services.
Payment Terms
Fees: Fees for our services will be outlined in a separate agreement. All fees are due as specified in the agreement. Late Payments: Late payments may incur interest charges and could result in suspension of service
Limitation of Liability
Your practice will not be liable for any direct, indirect, incidental, special, or consequential damages resulting from the use or inability to use our services. Our liability is limited to the amount paid for services rendered.
Indemnification
You agree to indemnify and hold [Your Medical Billing Company], its affiliates, officers, agents, and employees harmless from any claim or demand, including reasonable attorneys' fees, made by any third party due to or arising out of your breach of these terms and conditions or your violation of any law or the rights of a third party.
Termination
We reserve the right to terminate or suspend your access to our billing services with 30 days notice, for violation of these terms.
Governing Law
These terms and conditions are governed by and construed in accordance with the laws of St Louis MO. Any disputes arising under or in connection with these terms shall be resolved in the courts of MO.
Changes to Terms
We reserve the right to modify these terms and conditions at any time. Changes will be effective immediately upon posting on our website. Your continued use of our services following the posting of changes constitutes your acceptance of such changes.
